Re: S.W.A.C. Membership Application

Dear Union Member,

We appreciate your interest in a S.W.A.C. membership and have prepared the following outline to guide you through the individual membership application. We have also attached the individual application as well as the background criteria. Please review the attached prior to online entry so that you will know what will be required. You will also need a valid Visa, MasterCard, Discover or American Express for payment of your application. Please be advised that there will be two charges, one for the application fee of 134.37, and the other for the background screening fee. There is no set fee for the background screening and the cost can be driven by many different factors. Such as multiple addresses, AKA’s, background searches, etc. The average screening cost is $50.00 to $175.00.

If you would like to continue with the application process online, please follow the steps outline below:

1. Using internet explorer, please type in www.secureworker.com

2. Click the blue “Enroll or Renew now” option on the right hand side of the page.

3. Click the circle next to “Individual Membership” and continue.

4. Under application type—click the circle next to “Site Worker”,.

5. Under worker sponsorship you will see a grey highlighted box that says for self employed workers sponsoring their own application “click here”. An option will then appear for Self Sponsorship.

Click the circle and then enter your valid credit card information.

6. For security purposes you will be prompted to enter the text from the image that appears on your screen, then click continue.

7. Enter your social security number, date of birth and mother’s maiden name.

8. Under “Affiliation” click Union Member.

9. Under “Agency” click IBEW Local Union #164. Hit continue.

10. Fill in all of your personal information on the application.

11. **Identity Document Selection—chose any one from the top list for example birth certificate or passport. And any two from column A or one from Column A and one from Column B and click continue.

12. S.W.A.C. Processing Centers-Affiliation once again will be Union Member. Agency will be IBEW Local Union #164. Then select a processing center and click continue.

13. Membership Acceptance Form—after you have had a chance to fully review the application, then click “Yes I agree”.
