Apprentice Evaluation Forms

As of December 2023, the JATC has updated the on-line Apprenticeship Evaluation Form to be more user friendly and comprehensive when filling out an evaluation.  We request that you begin using the new on-line form versus the paper pdf form as the paper form will be phased out in 2024.
